When you beat the game on hard you have to load the save and complete the first level again and then the achievement will pop.

There's a Mafia II save editor in 360 Revolution. Since I'm not familiar with what that editor actually does, I've also uploaded a save that should get you what you want. I never ended up using it myself, though, so make sure you read the included notes and instructions.
