The only code I used was for the Crucible. I did not have any other codes/DLC/Preorder bonus. Specifically it was the code provided on the back of the manual titled "Your Crucible Pass Code"

Maybe someone out there needs this without some of the bonus/preorder DLC. I had to have the Crucible Pass as there are 3 achievements that require it.
The first save is right in front of the door...
